ADMISSION GUIDE

Pre-Admission
|
|
|
Your doctor and nursing team will make the necessary bookings for your admission
|
|
|
Kindly approach our Business Office Counters situated in the Hospital Lobby (Ground Floor) to confirm your admission
|
|
|
Room type and availability will be confirmed on the day of admission. If your preferred room type is unavailable, we will endeavour to offer you a transfer once your preferred room becomes available
|
|
|
Please remember to bring along all the necessary documents including your insurance card, claim forms or guarantee letters
|
|
|
For corporate and insurance clients, an appropriate deposit amount may need to be collected
|
|
|
For self paying clients, an 80% deposit of the estimated total cost of admission will be collected
|
|
|
|
|
|
|
Surgical Admission
|
|
|
You must fast(nothing to eat or drink) for at least 6 hours prior to your surgery
|
|
|
Please continue to take your usual medications(unless instructed to otherwise by your doctor)
|
|
|
You will be given instructions about admission timing based on the time of your surgery
|
|
|
|
|
|
|
Ambulatory Care Centre Admission Time

Please arrive at least 2 hours before your scheduled procedure time
|
|
|
You must arrange for an adult to accompany you home after your procedure if you require sedation or anaesthetic
|

DISCHARGE PROCEDURE
|
Our hospital discharge time is 12 noon. An additional half days’ surcharge may apply for late discharges. Discharges after 8pm will incur an additional full day’s charge
|
|
|
Please note that once your doctor completed the discharge notifications, it may take up to 2 hours to prepare your bill and medications. This may be longer if we have to confirm your the final bill with your insurance company
|
|
|
Discharge payments can be completed either at the Business Office located in the hospital lobby (ground floor) or in your hospital rooms (for in patients doing full card payments)
|
|
|
All charges must be paid in full upon discharge
|
|
|
If your deposit amount is greater than your hospital charges, a refund will be processed once the bill is finalised. If your deposit was made by credit card, it will be refunded to your credit card. For cash and cheque payments, a refund cheque will be issued
|
|
|
Please follow your doctor’s advice with regards to driving. Following some procedures, you may not be able to drive for days or weeks. You may be required to update your car insurer about your recent admission
|

All visitors and caretakers who exhibit symptoms are required to undergo a COVID-19 test at Kensington Green Specialist Centre before accessing patients in the Ward.
|
|
|
Children less than 12 years old are not allowed to visit the patient.
|
|
|
Only 2 (TWO) visitors per patient in the ward.
|
|
|
Only 1 (ONE) caretaker per patient in the ward.
|
|

Request for a Medical Report
|
Kensington Green Specialist Centre (KGSC) facilitates requests for medical reports for the following purposes:
|
|
- EPF / SOCSO
- Insurance Claim (Pay and Claim)
- Lawyer / Employer / Police Report / KKM
|
|
Patient's consent is compulsory for the release of the medical information. For patients under 18 years old, the consent of a parent or legally acceptable representative is required. If the patient is deceased or incompetent, consent is required from the next of kin of the patient or the legally acceptable representative appointed by the patient’s family member.

To ensure the smooth processing of your request, kindly ensure that the following documents are prepared:

- Photocopy of patient’s NRIC / Passport (front & back)
- Application & Consent for Release of Medical Information Form
- Necessary supporting documents where applicable (depends on type of request)
- If you are requesting on behalf of a patient:
- Photocopy of patient’s NRIC / Passport (front & back)
- Photocopy of representative’s NRIC / Passport (front & back)
- Application & Consent for Release of Medical Information Form
- Necessary supporting documents where applicable (depends on type of request)
- For Deceased Patient:
- Photocopy of spouse / third party’s NRIC / Passport (front & back)
- Photocopy of marriage certificate (if applicable)
- Photocopy of Deceased’s Death Certificate
- Grant of Probate / Letter of Administration (applies to requestors other than spouse)
- Application & Consent for Release of Medical Information Form
- Necessary supporting documents where applicable (depends on type of request)
- Example of necessary supporting document:
- Official Application Letter – Insurance Query Letter / Claim Form, Lawyer Letter, SOCSO Form, EPF Form, etc.
- Written Authorization Letter from patient (if collected by other than the patient / requestor)

Turnaround time for Complete Medical Report
|
|
The estimated turnaround for processing medical reports / insurance claims will be approximately 14 working days (Monday to Friday), from the time we receive all duly completed documents and payment.
Delay in processing may occur due to:
|
|
- Unavailability of Doctors / Consultants (e.g., away on leave)
- Patients having upcoming clinic appointments
- Patients are still admitted/hospitalized
- Patients with multiple reports requested from several doctors
- Limited number of sessions by doctors at KGSC (e.g. only two sessions per week)
- Visiting doctors without clinic sessions at KGSC

Receipt of Completed Medical Report
|
|
- Will I be notified when my report is ready?
- Yes, we will notify you regarding the availability of the report through phone/WhatsApp or email.
- You can collect the report personally at our Business Office counter located in our Lobby near the entrance during office hours. Alternatively, you can request to have the report posted to your provided address through a courier service (charges applicable) or emailed to the provided email address.
- Can I ask someone to collect the report on my behalf?
- Yes, please provide the consent form provided by us only or a written authorization letter (where applicable). This letter should clearly state your consent for the designated person to collect the report on your behalf.
- Can KGSC post it to my insurance company/SOCSO/EPF office?
- No, to maintain confidentiality and adhere to the PDPA Act 2010, we will only release the report to the requestor who has provided valid consent or authorization. This policy ensures the protection of your personal information and mitigates any potential disputes regarding proof of delivery. We appreciate your understanding and cooperation.

The fee schedule for the completion of Medical Reports is as per the table below, subject to periodic revisions. Additional charges may apply to specific reports, and you will be duly notified through email or phone call of any such adjustments.
